The Loden Group strives to provide it's clients with the highest quality providers.  We are  currently seeking Habilitation and Respite providers.  Descriptions of each position and the minimum state requirements are listed below.  If you are interested in habilitation, respite and/or attendant care employment with The Loden Group, you can download an application and the reference request form (you will need to print 3 reference forms).  Once we have received your application and references we will verify the information, then schedule an interview with you.

Eligibility For Employment

Federal law requires both new employees and re-hires to provide documentation of eligibility to work in the United States plus proper identity. Along with Arizona State standards for employment as stated late in this manual, a properly submitted form I-9 is required for employment.

All individuals wishing to provide consultation, habilitation and/or respite services on behalf of THE LODEN GROUP  must satisfy state mandated minimum requirements relating to the service class applied for along with requirements set forth by THE LODEN GROUP in order to be eligible for employment consideration.

 

The state mandated minimum requirements to be met before providing services include, but are not

 limited to the following (with some additional LODEN GROUP requirements):

1.    Must be at least 18 years of age    

 

2.    Fingerprinting, fingerprint clearance and disclosure statement

       regarding criminal record signed by applicant

3.    Current valid driver's license and reliable transportation

  

4.    Three references on LODEN GROUP forms

 

5.    Current CPR and first aid certification (original cards brought to

       office to be copied for file)

 

6.    Proof of training and proficiency in A.A.C. Title 6, Chapter 6, Article 9

      (given by The Loden Group during training)

 

7.    Evidence of basic client orientation and documentation proficiency

      (covered during initial training)

 

8.    Client intervention techniques (CIT) training; immediately if specified

       in the Individual Service Program Plan (ISPP)

 

9.    Knowledge, skills and experience sufficient to carry out the requirements

       of the position including NO LESS than 6 months experience working with children

 

10.   Evidence of successful completion of the Behavioral Architecture™ Training 

       Program which includes:

                    Behavioral Architecture™   - 8 hours

                    Documentation/Data            - 4 hours

                    Monthly Advanced Topic     - 2  hours (advanced topics offered monthly,     

                    providers must attend one (1) per quarter)

            

SERVICE DESCRIPTIONS:

Habilitation:

Per DDD definition, "this service provides a variety of interventions designed to maximize the functioning of consumers.  Services may include but are not limited to: habilitative therapies, special developmental skills, behavior intervention, and sensory-motor development.''

Respite

Per DDD definition, "this service provides short-term care and supervision consistent with the health needs of the consumer to supplement care to provide a safe living environment and/or support or relieve caregivers for the benefit of the consumer."

 

Attendant Care

Per DDD definition, "this service provides a certified and trained attendant to assist an individual to attain or maintain safe and sanitary living conditions and/or maintain personal cleanliness and activities of daily living.  The goal of this service is to assist the individual to remain in his/her home or participate in community activities."

 

Consultation

This position requires individuals to have been ABA habilitation providers for no less than 3 years and work with at least 5 different individuals with varying levels of challenges. Consultants must possess no less than a Bachelor's degree. Consultants are responsible for training individual programs to families and teams, reading and analyzing data and being able to answer behavioral questions.

 

Program Manager

This position requires individuals to have been ABA habilitation providers for no less than 2 years and work with at least 3 different individuals with varying levels of challenges. Program managers must be able to read and analyze data in order to move targets and programs along.
