The process to get started with
The Loden Group is an easy one...

If you have been referred to The
Loden Group and do NOT receive services through DDD, simply call the
office to make an appointment for an initial assessment
(602-381-0700).

First
- Call our office. We
will do an initial intake gathering general information (name, age,
diagnosis, address, services requested, etc...) and send you an
"Orientation Packet. Inside
of the orange folder you will find our client service policy and
several forms. The forms on the left side of the folder must be
completed and returned PRIOR
to
the start of any services.
The forms on the right side are a grievance form and
incident/accident form for you to keep should there be any issues
once services begin.
Please use the following checklist to help you with the
required paperwork (from orientation packet) so that we may
begin services for your child in a timely
manner:
I.Fill
Out and Return:
a.
Pre-Service Orientation Form (triplicate form, 2 pages, be sure to
sign all appropriate places on second page)
b. General
Consent
c. Release to
Share Information (list those individuals we are permitted to
speak to your child’s
program about...i.e. speech, teachers,
psychologist)
d. Schedule
Preference (fill out your child’s daily routine from the time they
awake until the time they go to sleep. Any
holes in the schedule are the times your child can do
habilitation)
SERVICES
CAN NOT BEGIN WITHOUT
THE ABOVE STATED FORMS COMPLETED, SIGNED AND IN OUR OFFICE.

(answer questions as completely as possible)
Initial assessment for habilitation can not take place until
we receive the two
forms stated above in our office.
II. Call
Support Coordinator:
a.
Let your support coordinator know you have chosen to receive
services through The Loden Group
b. Ask him/her
to send authorization for services to our office as soon
as possible.
Remember to ask for authorization for each service you intend
to use through The Loden Group (habilitation, respite, attendant
care)
c. If you do not
have the most recent copy of your child’s ISP (Individual
Service
Plan) request that as well
SERVICES CAN NOT BEGIN UNTIL AUTHORIZATION AND ISP ARE
RECEIVED FROM ddd
oNCE WE RECEIVE ALL OF THE ABOVE ITEMS, WE WILL NOTIFY YOU TO
SET UP AN INITITAL ASSESSMENT.
fOR THOSE CLIENTS NOT RECEIVING hABILITATION SERVICES, WE
JUST NEED THE REQUIRED PAPERWORK. nO ASSESSMENT IS
NEEDED.
III. For
those clients utilizing habilitation services, send copies
of:
a.
Your child’s most recent ISP
b. Any current
progress reports from other interventions (developmental
paediatrician, speech, OT
Please
call us with any questions regarding required paperwork. Services
CAN
NOT begin until paperwork and authorizations are received in our
office. Once again, we are pleased you have chosen The Loden Group
and we look forward to helping your child and family.
