Some of you contacted our
office just a few days ago and others contacted our office many
months ago. We want to
ensure you that we are working diligently to find providers for your
children as quickly as possible. We have ongoing recruiting
practices, which include monthly ads placed in the newspaper, online
services and flyers at all community colleges and universities
within Maricopa County. We pride ourselves on
placing only qualified individuals, who we feel will be responsible,
dedicated and have the desire to make a difference in the life of a
child. For these
reasons, we will not hire “just anyone.” Our screening process is an
attempt to make certain that only people who are truly dedicated to
the cause will be hired.
It is our policy to include
any new family and any current family that is in need of providers
on our match list. Once
a provider has been matched, we contact the family with the
provider’s contact information. This has lead to periods
where families are unable to contact the provider in a timely
fashion. Below you will
find a contact release form.
If you complete the form, you are authorizing our agency to
give a potential provider your contact information, once a match has
been made. This way,
both the family and the provider has the means to contact each
other. If you do not
complete the form, we will not release your contact
information. We will
contact you with the provider’s contact information; however, if the
provider has not heard from a family within 48 hours, we will move
onto the next waiting family.
An issue that had arisen in
the past is the time between finding a provider and scheduling an
assessment in a timely manner.
In order to schedule an assessment, we must have all of the
necessary pre-service forms, a copy of the most current ISP and
authorization of hours.
Without these three components, we are not able to schedule
an assessment. We
have provided the pre-service forms at the bottom of this
page. Please print,
complete and sign these forms and mail them back to the office. We will sign off on the
forms and mail your copies back to you.
We believe these efforts will
improve the overall set-up procedure. We would like to thank you
for your patience as we make efforts to find a qualified provider
for your child.
