Habilitation – Traditional
Program
Service
Goals
To enable the consumer to acquire
knowledge and skills and be a valued member of his/her community
based on his/her own choices.
To provide training to increase or
maintain the consumer’s self-help, socialization, and adaptive
skills to reside and participate successfully with his/her family in
his/her own community.
To assist the consumer in achieving and
maintaining a quality of life that promotes the consumer’s vision of
the future.
Method of
Implementation
The Loden Group will meet with client and
parent(s)/guardian(s) in our office for a one hour assessment with a
consultant/program manager. Assessment will include family input on
priorities and goals and hands-on with client as well as ISP
goals. Assessment will NOT take place until The Loden Group office
has received authorized service hours through the FOCUS system, and a copy of your child’s current
ISP.
Based on the assessment and in
conjunction with ISP goals, The Loden Group will establish a
habilitation program with written strategies for each skill to be
worked on.
The Loden Group will mail the program to
the family within 10 days of
assessment.
The Loden Group will send a Senior
Provider or Program Manager out to the client’s home for initial
program implementation support. The Loden Group This will be a
60-minute training to go over the program and help support the
providers and family in strategies for implementation of the
program. The time line for this meeting will currently be within 30
days of mailing of program. As we identify and hire more qualified
staff, our goal is to provide this support meeting within 10-14 days
of program mailing.
Families as always have the option to
attend The Loden Group’s main 12-hour training that providers go
through and all monthly topics that are offered. These trainings are
offered free of charge to two members of each client family and all
Loden Group providers.
For families utilizing 60 or more hours
of habilitation per month, we will offer a choice of 2 or 4
follow-up meetings throughout the year. For families willing to come
to our office, we will provide quarterly support meetings. For
families choosing home-based support meetings (beyond the initial
which is always in-home), we will send out a program manager
semi-annually to review and update the
program.
Program goals will coincide with ISP
goals and goal dates. Goals are written for one year, but can be
updated any time throughout the year based on mastery of skills.
Recordkeeping and
Reporting Requirements
The habilitation provider shall submit
monthly progress reports, including a written summary describing the
specific service activities and the performance data that identifies
the client’s progress toward achievement of the established
objectives. The required data will take the form of
opportunity/response data sheets kept at the family home and program
summary sheets (submitted with report). THE LODEN GROUP will send
report to consumer’s support coordinator within ten business days of
the close of the month.
THE LODEN GROUP must maintain on file
proof of hours worked by their direct service staff, e.g., staff
time sheets. Each
document must be initialed, dated and signed by the
consumer/family/consumer’s representative as verification of hours
served.
The LODEN GROUP will send to the Support
Coordinator the goals (program objectives) and training strategies
upon within 10 business days of program
implementation.
